Wilco Peul is sinds 2008 hoogleraar neurochirurgie, afdelingshoofd van de gelijknamige afdeling in het LUMC en voorzitter Universitair Neurochirurgisch Centrum Holland (UNCH), een Coöperatie van het LUMC samen met het HMC en HagaZiekenhuis. Vanuit het UNCH-bestuur wordt de neurochirurgie gecoördineerd in 8 ziekenhuizen in Leiden, Den Haag, Haarlem, Schiedam, Delft, Gouda en omstreken, met als doel een kwalitatief hoogwaardige en gelijkwaardige behandeling te realiseren met meerwaarde voor alle patiënten door concentratie van gespecialiseerde zorg per ziekenhuis. Vanaf 2015 is hij actief in de UEMS Neurochirurgie te Brussel namens de European Association for Neurosurgical Societies (EANS), waarin hij samen met de secties Orthopedie en Neurochirurgie een Europees Training Curriculum voor de spinaalchirurgie ontwikkelt. Vanaf 2020 leidt hij als president van de UEMS sectie Neurochirurgie de ontwikkeling van het nieuwe Europese opleidingsprogramma. Sedert 2021 is hij Editor-in-Chief van ‘Brain and Spine’, een recent opgestart wetenschappelijk tijdschrift van 2 wetenschappelijke verenigingen, de EANS en Eurospine, in samenwerking met uitgever Elsevier. Buiten het LUMC, maar wel vanuit zijn rol als klinisch onderzoeker, is hij in 2020 benoemd tot bestuurslid van ZonMW voor een periode van 3 jaar.

Neurochirurgie
Doelmatigheid van neurochirurgie in onderzoek, patiëntenzorg en onderwijs staat bovenaan op de Leidse en (inter)nationale agenda van de hoogleraar neurochirurgie en zijn staf. Dit uit zich in richtlijnveranderingen door onderzoek in spinale chirurgie en neuro-traumatologie, in het bijzonder internationaal vergelijkend onderzoek naar diagnostiek en behandeling van ernstig hersenletsel en dwarslaesie. Tevens is hij namens de wereldorganisatie betrokken bij onderwijs in Laag-en-Midden-Inkomen-Landen (LMICs).
In de breedte van het volledige palet aan neurochirurgisch te behandelen aandoeningen faciliteert de leerstoel de verschillende onderzoekslijnen Wervelkolomgeneeskunde, Zenuwchirurgie, Translationeel Neuro-Oncologie-onderzoek, Schedelbasischirurgie, Functionele Chirurgie en Acute Neurochirurgie.
Momenteel (2021) start hij vanuit zijn leerstoel onderzoek op naar de voor patiënten meest optimale en effectieve behandeling voor patiënten met tumoren van Axiale Skelet (schedel & wervelkolom), hetgeen past bij de chirurgische expertisen van het LUMC-profiel, dat van de afdeling en nevenafdelingen Oncologie, Orthopedie, Pathologie en Radiotherapie, inclusief het Holland Proton Beam centrum. In die hoedanigheid heeft hij zitting in de Nederlandse Commissie voor Beentumoren.
Wetenschappelijke carrière
Wilco Peul heeft zich al jong tot doel gesteld bestaande en nieuwe interventies kritisch te evalueren in gerandomiseerd onderzoek. Als opgeleid epidemioloog is hij in 2008 cum laude gepromoveerd op zijn vergelijkend kosteneffectiviteitonderzoek ‘The Sciatica Trial’ naar vroege herniachirurgie versus een langer afwachtend beleid met eventueel latere ingreep. Deze methodologie, nieuw in de neurochirurgie, heeft daarna geleid tot meerdere vergelijkbare klinische onderzoeken, welke het beleid van spinale zorg voor hoogvolume-aandoeningen heeft gewijzigd. Zijn oratie in 2010 ‘Spiegel der Rationele Zorg, Van timmermansoog naar computergenavigeerde Neurochirurgie‘ geeft dan ook het beeld van innovatie, evaluatie en doelmatigheid van zorg met uitkomstmetingen over het gehele spectrum van aandoeningen met neurochirurgische zorgvraag behandeld in de groot Leidse regio.
Prijzen en eervolle benoemingen
2008 ISSLS Best Paper Award in Hong Kong voor zijn studie naar doelmatigheid van lage rugherniachirurgie, gepubliceerd in NEJM.
In 2015 en 2016 is hij in Toronto, Canada, benoemd tot Visiting Professor ‘Spine Surgery’.
In 2019 Visiting ‘Set Rengachary’ Professor Neurotraumatology New Delhi, India.
In 2020 benoeming tot Editor-in-Chief van het nieuwe in 2021 te lanceren Elsevier-tijdschrift ‘Brain & Spine’, het officiële tijdschrift van de European Association of Neurosurgical Societies & Spine Society of Europe.
